Medication adherence in mental health
challenges and possibilities of technology use in a public hospital

Objective: To discuss barriers related to medication adherence in psychiatric patients and analyze the potential role of digital technologies in supporting rational medication use in low-resource settings. Methodology: Experience report from clinical pharmacy in a psychiatric hospital combined with a narrative literature review in SciELO, LILACS, and MEDLINE databases between 2015 and 2025, using the DeCS/MeSH descriptors Medication Adherence, Mental Disorders, Patient Education as Topic, and Health Technology. Development: Low adherence is related to multiple factors, including adverse effects, stigma, lack of understanding about treatment, and external influences. Evidence shows that digital interventions can improve adherence, but barriers such as digital inequality hinder implementation in public hospitals. Simple and accessible technologies emerge as viable alternatives. Final Considerations: The use of digital tools can strengthen the role of the clinical pharmacist and the multiprofessional team in promoting the rational use of psychotropic drugs and improving medication adherence in mental health, even in contexts of structural limitations.
